Here are some of the registration forms that you'll need to fill out for kids signing up at the Comstock Co-op Preschool:

Co-op Guidelines	- a brief introduction to the way that the Co-op works.
Health Appraisal Form	- to be filled out by child's physician.
"Potty" Form	- to be filled in and mailed to Kalamazoo Family & Independence agency by any parent/guardian who plans on working in the classroom.
Field Trip Form	- to be filled out by the parent/guardian.
Morals Statement	- to be filled out by the parent/guardian.
Registration Form - 3's	- to be filled out by the parent/guardian.
Registration Form - 4's	- to be filled out by the parent/guardian.
